Master Boxing Information
Lasik: Boxer who wish to compete after Lasik surgery are required to have attached to their passbooks a completed”Release to Compete After Lasik Surgery form. This form requires the written approval of their ophthalmologist. Boxers competing after LASIK surgery will not be covered under the USA Boxing Insurance program for damage to the eyes.

Release to Compete With Braces
USA Boxing Technical Rule 5.1.3.1. Boxers who wish to compete with braces are required to have attached to their passbooks a completed Release To Compete With Braces form. This form requires the written approval of their dentist, parents and/or guardian (if under 18 years of age) and a dentist-molded mouthpiece. This includes upper and/or lower braces. Boxers competing with braces waive the right to dental coverage under the USA Boxing insurance program.

United States Amateur Boxing’s insurance company requires a signed medical treatment form either authorizing emergency medical treatment or not authorizing medical treatment for all participating individuals. Participants under 18 years of age, are required to have parental / guardian signatures as well.

2023 NC AZALEA FESTIVAL ANNUAL BOXING CHAMPIONSHIP INVITATIONAL FOR INTERNATIONAL BOXERS COMING FROM OUTSIDE THE UNITED STATES

Held under the sanction of USA Boxing, Inc. North Carolina Friends of Boxing, Inc. 4613 Whiteweld Terrace Dr. Wilmington, NC 28401 To: International Amateur Boxing Coaches, Boxers, Team Manager, & Doctors This letter serve as an official invitational for amateur boxers, who are coming from outside the United States, to participate in the 2022 NC Azalea Festival and may also be used for visa request. Elite male amateur boxers are eligible to compete at the 2021 Azalea Festival Boxing Tournament if they provide a letter/travel permit from your local AIBA-registered boxing federation. The letter must list the specific names of each boxer and state they are approved to compete in the 2019 NC Azalea Festival Annual Boxing Tournament with headgear.
Every international club & athlete, including Canada, is required to obtain a written authorization letter from their Federation/Association to participate. This authorization letter must be provided to NC Azalea Festival/Friends of Boxing, Inc. with all the participants’ entry forms, so we can complete reporting obligations to USA Boxing. All international athletes and coaches must have a current-dated International or Federation/ Association passbook. Insurance for all 2 competition date is required through your Federation/Association. The proof of your federation/association insurance, or request for insurance, must be provided before March 3, 2022 . If your federation does not provide insurance, you must be insured through TravMed, an Olympic insurance program, at a cost of $5.00 per day, per person. Athletes must be covered and coaches should be covered. Additional members of your delegation may also wish to carry this medical insurance at the same cost. It is suggested you insure I for every day of travel in addition to date’s competition.
