• First & Last Name of Boxer
• Boxers date of birth (if USA Boxing has NOT verified age, send a picture of birth certificate or passport)
• Boxers weight
• Boxers Amateur Record (wins/losses)
• Name of Gym and City, State
• Trainers/Coaches Name
• Trainers phone number

Potential match up info will be provided to the Trainer/Coach as we near the show date.

Each Trainer will receive the following information for match approval:

• Opponent name
• Age & Weight
• wins/losses

*Once you reply accepting the match up, you will be contacted with weigh-in instructions, corner assignment and bout order.*
