REQUIRED FOR REGISTRATION:

• First & Last Name of Boxer
• Boxers date of birth (if USA Boxing has NOT verified age, send a picture of birth certificate or passport)
• Boxers weight
• Boxers Amateur Record (wins/losses)
• Name of Gym and City, State
• Trainers/Coaches Name
• Trainers phone number

Potential match up info will be provided to the Trainer/Coach as we near the show date.

*Each Trainer will receive the following information for match approval:

• Opponent name
• Age & Weight
• Opponents home state
• wins/losses

*Once both sides accept a match, you will receive confirmation of matching. 3 days prior to the event you will receive bout order and corner assignment.*
